What you will do:
Conduct assessments of clients in their homes or other settings to identify their individual needs.
Develop tailored reablement plans to enhance clients' independence and quality of life.
Collaborate effectively with other healthcare professionals to ensure a cohesive approach to patient care.
Continuously monitor and evaluate clients' progress and adjust care plans as necessary.
Provide advice, education, and support to clients and their families to promote long-term health and well-being.
Requirements:
HCPC registration is essential.
You should have UK experience working as an Occupational Therapist.
A degree or equivalent in Occupational Therapy is required.
Possessing a valid full UK driver's license is necessary, as you will need to conduct visits to clients in their homes or other settings, such as hospitals.
