Social Worker (Level 3) – Hospital Integrated Care and First Discharge Team
Location: Whiston Hospital, Halton, UK
Salary: £35 per hour, ongoing
Position: Full-time and Part-time opportunities available
Halton Council is seeking a skilled Social Worker to join the Hospital Integrated Care and First (HICaF) Discharge Team at Whiston Hospital. This vital role is key to ensuring the safe and timely discharge of patients, particularly during peak periods such as winter, to meet the increasing demands of the Acute Trust.

Key Responsibilities:
- Collaborate with a multidisciplinary team to ensure safe, efficient, and timely discharge of patients.
- Manage a complex caseload independently, ensuring high-quality care and timely interventions.
- Provide essential support and assistance to individuals and families transitioning from hospital to community-based care.
- Liaise effectively with healthcare professionals to ensure seamless, person-centred care for patients.
Requirements:
- Proven experience within an Integrated Hospital Discharge Team.
- Desirable: Familiarity with Eclipse (though not mandatory).
- Ability to thrive in a fast-paced environment and manage tasks efficiently.
- A full driving licence and access to a vehicle are essential.
Qualifications:
- A degree in Social Work or a related field is required.
- Strong communication skills, including accurate documentation and reporting.
- Ability to work independently and as part of a team.
- Experience in mentoring or supervising others is a plus.
- A commitment to continuous professional development and keeping current with best practices in social work.
