Social Worker - Adults Substance Misuse in Scunthorpe UK, earning £35 hourly. This post is for 6 months with a view to extend another 6 months. Here is an exciting opportunity for a Specialist Social Worker who has the expertise in Substance Misuse. You will join the Adult and Health service area, specifically in the Mental Health and Disability Services Division. This role is full time, and you will be part of a dynamic team that aims to provide effective specialist interventions for working age adults dealing with substance misuse issues. Your role will involve managing complex cases and coordinating care within the local service area.

What you will do:
- Design appropriate care packages following the department’s Care Management systems, ensuring compliance with UK NICE Guidelines and relevant legislation.
- Conduct assessments and re-assessments of clients' needs, implementing community recovery and aftercare packages while evaluating their effectiveness.
- Handle referrals, undertake emergency visits, and complete necessary risk assessments.
- Establish and maintain partnerships with healthcare colleagues and service providers to address the complex needs of service users.
- Make informed professional decisions on individual cases and provide supervision to local Substance Misuse Services.
- Be an active member of multidisciplinary teams to formulate and implement care plans, promoting recovery and social inclusion.
- Liaise with various Health and Social Care services to plan and monitor a range of interventions.
- Maintain accurate entries on case management systems and provide monthly reports to treatment services and commissioners.
