What you will do:

• Conduct community visits to assess patients recently discharged from hospitals ensuring a smooth transition back to home life.
• Develop individualised physiotherapy plans tailored to the specific needs of each patient.
• Collaborate effectively with other healthcare professionals to provide holistic care.
• Motivate and educate patients on exercises and recovery best practices to promote their independence.
• Regularly monitor and report patient progress making adjustments to treatment as necessary.

Requirements:

• Applicants must be registered with the Health and Care Professions Council HCPC and have a minimum of two years of experience in a relevant field.
• A minimum of 2years of experience in community Physiotherpist is preferred.
• Possessing your own transport is essential to perform community visits efficiently. We are seeking someone who is flexible adaptable and eager to face new challenges head-on.
