This is an exciting opportunity for an enthusiastic and skilled Community Mental Health Social Worker. Join the vibrant Stepney and Wapping Neighbourhood Mental Health Team, a pivotal part of the East London Foundation Trust. In this role, you will actively support service users on their personal recovery journeys. This well-established team works as an integrated multi-disciplinary unit, combining efforts from staff in the East London NHS Foundation Trust and the London Borough of Tower Hamlets. You will be part of a team dedicated to delivering high-quality services to individuals with severe and enduring mental health needs, in line with the Transformation Project. Emphasis is placed on partnerships with service users, carers, local authorities, and various voluntary groups. If you have a minimum of two years of experience and are interested in working full-time at 35 hours per week, we would love to hear from you.

What you will do:
- Work collaboratively within the Stepney and Wapping Neighbourhood Mental Health Team to provide high-quality mental health services.
- Support service users through their personal recovery journeys, fostering independence and empowerment.
- Engage with carers, local authorities, voluntary groups, and other partners to enhance service delivery and integration.
- Conduct assessments and develop care plans suited to individual needs, focusing on goals and positive outcomes.
- Maintain comprehensive records and reports that meet professional standards.
