What you will do:

• Work within a multidisciplinary community social care team to provide high-quality services for individuals with complex needs.
• Conduct assessments, support planning, and reviews to ensure individuals receive the necessary long-term support.
• Carry out Mental Capacity Assessments and contribute to shaping personalised care plans.
• Manage an allocated caseload within agreed frameworks to meet performance and quality standards.
