Qualified Social Worker – Adults Front Door Team

Position: Full Time/Part Time (36 hours per week, Monday to Friday Hybrid)
Are you an experienced Qualified Social Worker seeking a new and rewarding challenge? We have an outstanding opportunity for you to join the Adults Front Door Team within Adults Social Care in Knowsley. This role involves handling complex cases while earning a competitive £31.15 per hour in an ongoing capacity. With the flexibility of hybrid working, you will work 36 hours per week from Monday to Friday Hybrid work, including at least two days in the office. Both full-time and part-time applications are welcome!
As a vital member of the team, you will be responsible for conducting Social Care Needs Assessments (SCNA), Mental Capacity Assessments (MCA), safeguarding, and risk assessments. Your focus will be on short-term interventions to help service users maintain their independence, preventing unnecessary reliance on services. You will also collaborate closely with Rapid Response and other professional services in a fast-paced environment.

What You Will Do:

Conduct Assessments: Perform Social Care Needs Assessments (SCNA) to identify and address the support needs of adults.

Mental Capacity Assessments (MCA): Carry out comprehensive MCA evaluations and take appropriate action based on the outcomes.

Safeguarding: Lead on safeguarding cases and ensure all interventions are compliant with statutory guidelines.

Risk Management: Complete risk assessments and develop risk management plans to deliver effective support to service users.

Collaboration: Work closely with the Rapid Response team and other multi-agency professionals to provide seamless, coordinated care.

Short-Term Interventions: Implement and monitor short-term intervention plans to empower service users and avoid long-term dependency on services.
