What you will do:
- Conduct detailed assessments of patients with complex care needs, utilising various diagnostic tools and methods
- Develop, implement, and evaluate specialised care programmes tailored to meet individual patient requirements
- Undertake comprehensive risk assessments and create risk management plans
- Work autonomously to diagnose and provide specialist assessments for addiction and adult mental health needs
- Serve as a named care coordinator, managing care packages and coordinating multi-disciplinary interventions
- Lead and participate in multi-agency meetings, contributing to service evaluations and policy developments
- Communicate effectively with patients, their families, and other healthcare professionals to ensure cohesive care delivery
- Provide professional guidance to junior staff and act as a role model, promoting high standards of practice
- Deliver training and education to staff and students, ensuring a culture of continual professional development
- Keep detailed clinical records, complying with both policy and professional standards
- Reports to: Community Drug and Alcohol Team Manager (CDAT).
