What you will do:

  • Provide professional recommendations and support to families and caregivers, empowering them with knowledge and strategies for continued care.
  • Maintain accurate records and documentation to track patient progress and treatment effectiveness.

To excel in this role, you must be registered with the Health and Care Professions Council (HCPC) and possess a valid UK driver's licence to travel to various community sites within Brentford.
