What you will do:
- Undertake initial assessments within your area of specialism, providing a crucial first point of contact for those in need.
- monitor, and review care plans, ensuring effective collaboration with users, carers, and other professional agencies.
- Work in accordance with the council’s Financial Regulations and explore all available resources for service users.
- Comply with recording policies, completing necessary forms and documents to maintain accurate records.
- Utilize information management systems for effective data recording, retrieval, and analysis.
- Receive guidance and supervision from your line manager and work towards meeting agreed standards and targets.
- Adhere to all council policies and procedures in your duties and any other tasks as requested by management.
