**What you will do:**

• Lead a dedicated team to create and implement effective support plans for individuals with learning disabilities and autism.
• Coordinate and manage diverse settings, ensuring 24-hour support is in place.
• Work with a multi-disciplinary team and associated agencies to deliver person-centred care.
• Perform assessments, develop care plans, and manage crisis interventions independently.
• Supervise and mentor junior staff, fostering a positive and supportive team culture.
• Maintain accurate and thorough records in line with regulatory and organisational standards.
• Participate in health and wellbeing activities with service users, fostering an inclusive environment.
• Oversee and administer medication according to policy guidelines.
• Attend to shifts according to the Duty Roster and be adaptable to changes in shifts when needed.
