We are seeking a skilled and dynamic Band 5 Team Leader to join our vibrant team in the beautiful island of Guernsey. What you will do:
- Coordinate and lead a team to deliver exceptional care and support to individuals with learning disabilities and/or autism.
- Develop, implement, and evaluate personalised programmes of care and support, using evidence-based practices.
- Autonomously manage crises as they arise, ensuring the well-being of service users.
- Maintain meticulous records of care provided, in line with professional standards and guidelines.
- Facilitate person-centred activities, from recreational outings to daily living tasks, promoting active participation and social inclusion.
- Conduct risk assessments to maximise opportunities for service users in a safe manner.
- Manage medication in accordance with relevant policies.
- Ensure compliance with organisational and health and safety policies, contributing to a secure environment for all.
- Provide leadership to junior staff, fostering a collaborative and supportive team culture.
- Undertake rota management, ensuring efficient and effective operation of services.
Essential:
- Registered Health or Social Care Professional (e.g., Nurse, OT, SLT, Social Worker, RBT) or
- Degree in Health & Social Care with experience supporting individuals with learning disabilities and/or autism
- Strong IT, verbal, and written communication skills
- Leadership ability and experience
- Caring, empathetic, and person-centred approach
- Valid driving licence (flexible based on location)
- Good understanding of learning disabilities and/or autism
- Familiarity with person-centred care and Positive Behavioural Support (PBS)
