What you will do:

• Provide high-quality occupational therapy for individuals within the community rehabilitation setting, focusing on early discharge and maintaining independence.
• Conduct assessments and create individualized treatment plans for community rehabilitation and equipment provision.
• Offer guidance and support to clients in reviewing care packages, ensuring they meet the highest standards.
• Work collaboratively with a multidisciplinary team to ensure comprehensive and cohesive patient care.
• Engage in documentation and reporting on patient progress to facilitate successful rehabilitation outcomes.

Requirements:

• Registered with the HCPC.
• A minimum of two years' experience in community rehabilitation and early discharge.
• Skills in equipment provision and the ability to review care packages effectively.
• Car driving is preferable.
