If you have expertise in Early Discharge and Community Rehabilitation, as well as skills in equipment provision and review of care packages, this could be the perfect role for you. Ideally, you should hold an HCPC registration and possess a driving licence.

What you will do:

• Assist qualified therapists in delivering individualised therapy sessions.
• Support patients with early discharge and community rehabilitation plans.
• Coordinate the provision and maintenance of therapy equipment.
• Collaborate in the assessment and review of patient care packages.
• Document and report patient progress to the qualified clinical team.
• Engage and motivate patients throughout their therapy journey.
