We have an incredible opportunity for an Adult Social Worker to join our dynamic Integrated Adult Community Care Team on the picturesque island of Guernsey. What you will do:
- Provide a social work service to adults in Guernsey, conducting assessments and coordinating services for those wishing to remain in their homes.
- Formulate accurate assessments of complex psychosocial needs and contribute towards holistic assessments within the multi-professional team.
- Offer specialist advice and support to the team on social care issues related to adults, advocating for service users and their families around complex emotional and practical needs.
- Carry out person-centred assessments and manage a dynamic caseload with a high level of autonomy, ensuring timely and accurate reporting.
- Network with local community resources to advocate for care offered to service users and maintain relationships with partners in health, mental health agencies, and other services.
- Participate in team meetings, training, and development programmes to keep abreast of best practices and support the induction of new team members and students.
