Job Title: Qualified Social Worker - Adults

We have an exciting opportunity for a Qualified Social Worker within our Adults Specialism in Crewe, UK. This role offers a competitive hourly rate of £35.91 on a full-time basis over a 3-month period. These are full-time positions and candidates need to be in a commutable location as many assessments are face to face. The role requires experience completing DSTs and a good knowledge of the framework.

What You Will Do:
- Undertake comprehensive assessments of vulnerable adults' needs, considering their physical, emotional, and social well-being.
- Develop and implement individualized care plans that promote independence and choice.
- Regularly review and evaluate care plans to ensure they meet the changing needs of service users.
- Identify and respond appropriately to safeguarding concerns, following local policies and procedures.
- Collaborate with individuals, families, and other agencies to minimize risk and promote safety.
- Manage a complex caseload, prioritizing work effectively and meeting deadlines.
- Maintain accurate and up-to-date case records using relevant database systems.
- Work closely with health professionals, voluntary organizations, and other agencies to ensure a coordinated approach to care.
- Participate in multi-disciplinary meetings and contribute to joint planning.
- Apply the principles of the Mental Capacity Act in all aspects of practice.
- Conduct capacity assessments and make best interest decisions when required.
- Provide supervision and mentoring to trainee social workers and less experienced colleagues if required.
