This is an exciting opportunity! North Somerset Council's Hospital Discharge Assessment Team is eagerly seeking two experienced social workers to join their dynamic team amidst the unique challenges of hospital winter pressures. Your role will be pivotal in supporting adults ready for discharge from hospital who have care and support needs. As part of this committed and friendly team, you will work primarily from home, with necessary visits to Weston-super-Mare Town Hall for support, supervision, and team gatherings. The position is initially for three months, during which your expertise will be crucial in executing effective assessments and providing superb community-based support.

What you will do:
- Conduct assessments both in the community and within acute hospitals for adults ready for discharge.
- Collaborate with community health providers and local acute trusts to ensure seamless transitions for discharge patients.
- Manage a fast-paced workload with quick turnover, emphasising short-term interventions.
- Maintain precise case notes and records using shared case management systems such as LAS to ensure up-to-date information and team communication.
- Engage with the community through follow-up reviews after initial assessments.
- Travel throughout Northern Somerset, requiring the use of your own vehicle, to reach clients in rural areas not served by public transport.
