What you will do:
- Conduct independent triage and assessments to facilitate safe patient discharges.

- Implement your knowledge and experience in the IDH/D2A Model to improve patient outcomes.
- Collaborate with a multidisciplinary team to deliver high-quality care.
- Utilise your experience as an occupational therapist or physiotherapist to innovate and improve clinical practices.
To qualify for this exciting locum opportunity, you must be a registered HCPC professional with at least two years of experience. A car driver is preferred to facilitate home visits, ensuring you can deliver the best possible care to your patients.
