Social Services Occupational Therapist within Reablement in Brent (UK), earning £25.44 hourly. Full Time – part time will be considered for this role, which is based at multiple sites/offices. What you will do:
- Provide support to individuals in regaining their independence following hospital admission or after experiencing health deterioration.
- Work closely with a multidisciplinary team to develop and implement personalised reablement plans.
- Assess client needs, adapting therapeutic interventions to help them achieve their personal goals.
- Engage with clients and their families, ensuring effective communication and understanding of reablement processes.
- Regularly review and update care plans to reflect clients' progress and changing needs.
- Offer advice and education to clients and carers, promoting continued independence.
- Provide mentoring and support to less experienced team members, fostering an environment of learning and growth.
The ideal candidate will be HCPC registered, ideally with two years of relevant experience. A driving licence is preferable to facilitate travel between sites.
