What you will do:
As an Adult Social Worker, you will be responsible for:
- Delivering a high-quality casework service to adults within the community by carrying out thorough assessments and coordinating services for those wishing to remain at home or requiring care transitions.
- Offering specialist advice and consultation within the Integrated Adult Community Care Team, ensuring people-centred support is provided.
- Supporting individuals with complex psychosocial needs, including those with life-threatening illnesses, by offering advocacy and emotional support to them and their families.
- Managing an active caseload, responding to referrals, and prioritising work efficiently to meet the varied needs of service users.
- Collaborating with a multidisciplinary team to develop holistic care plans, participating in weekly meetings to review and adjust support as necessary.
- Engaging with local community resources to ensure comprehensive support for service users, including networking with health agencies, private care providers, and benefit offices.
- Supporting the training and development of less experienced social workers and students, promoting a culture of learning and continuous improvement.
