Qualified Social Worker - Band 6 - Practitioner within Adult Disability and Mental Health in Guernsey.
Salary: £73,085 Annually.

What you will do:
- Provide a high-quality, research-based casework service to adults residing in Guernsey.
- Carry out assessments and participate in coordinating services to support individuals in their own homes or in transitioning into care.
- Engage flexibly with a diverse range of service users to adapt to their changing needs.
- Offer specialist social work advice and consultation within the Adult Community Health and Social Care Team.
- Advocate for service users by supporting and representing their psychosocial and practical needs, particularly those with life-threatening illnesses.
- Conduct person-centred assessments, creating and reviewing care plans in partnership with multidisciplinary teams.
- Ensure compliance with safeguarding policies and address the needs of adults at risk.
- Mentor and support less experienced social workers, support workers, and social work students.
