What you will do:
- Deliver skilled occupational therapy interventions directly in the homes of residents.
- Manage cases with an emphasis on outcome-focused reablement plans lasting up to six weeks.
- Conduct comprehensive assessments to determine both clinical and personal needs.
- Develop and implement intervention plans, including advanced movement and handling assessments.
- Order and provide necessary adaptive equipment to facilitate independent living.
- Partner with a multidisciplinary team to ensure seamless service delivery.
- Maintain detailed case records using the Mosaic system, ensuring accuracy and thoroughness.
To excel in this role, you should be an HCPC-registered Occupational Therapist with experience in adult social care or community settings. Proficiency in IT and familiarity with the Mosaic system will give you an edge. Strong case management skills are crucial, as is the ability to thrive in a fast-paced, multidisciplinary team environment.
