You will be working on an ongoing basis, providing specialized care to individuals living with dementia and enduring mental illnesses. What You Will Do:

• Assess, develop, implement, and evaluate care plans using evidence-based standards.
• Operate autonomously and know when to escalate care concerns.
• Provide guidance and direction to other staff in care delivery.
• Support patients in daily living activities as outlined in care plans.
• Act as a health promoter and educator for patients and their networks.
• Report incidents and participate in quality and risk management processes.
• Maintain therapeutic relationships and respect communication needs.
• Collaborate effectively with multi-disciplinary teams.
• Serve as a professional role model and take charge when needed.
• Employ strong planning and organisational skills to manage tasks and resources.
