What you will do:

• Conduct comprehensive assessments of stroke rehabilitation patients to develop and implement personalised care plans.
• Collaborate with a multidisciplinary team to ensure holistic and effective patient care.
•  Provide education and support to patients and their families to optimise recovery and maintain quality of life.
•  Utilise evidence-based practice to enhance patient care outcomes.
•  Document patient progress accurately and adjust treatment plans proactively.

Requirements:

•  HCPC registration is essential.
• At least 2 years of experience in stroke or neuro rehabilitation, preferably within the NHS.
•  A car driver is preferable, facilitating easy travel between our multiple sites.
• Familiarity with working full-time shifts of 08:00 to 16:00.
