What you will do:
Assess and plan safe and timely discharges from hospitals, ensuring all processes follow relevant legislation and policies. Conduct Care Act assessments and Mental Capacity Act assessments when required to determine appropriate patient support. Collaborate with NHS teams, community services, and care providers to ensure seamless support for patients post-discharge. Identify and arrange suitable care packages, rehabilitation, or reablement services tailored to individual needs. Provide professional supervision and guidance to junior social workers or support staff, enhancing team effectiveness. Maintain meticulous records and reports that comply with local authority and legislative requirements. Advocate for patients’ rights, ensuring they are active participants in their care planning and transitions. Engage in safeguarding investigations and multi-disciplinary meetings, contributing to continuous improvement in care delivery. Stay informed about changes in legislation, policies, and best practices in adult social care, embedding this knowledge in your work.
