What you will do:

•  Conduct comprehensive assessments of needs and risks, creating collaborative care plans with service users, their carers, and other professionals.
• Offer expert guidance, supervision, and mentorship to social workers and team members to uphold high standards of practice.
• Collaborate closely with health professionals, voluntary organisations, and community resources to deliver integrated care.
• Lead safeguarding enquiries, ensuring alignment with policies and legislation such as the Care Act 2014 and the Mental Capacity Act 2005.
• Champion a strengths-based and person-centred care approach, placing service users’ voices at the forefront of all decisions.
• Support the integration of evidence-based practices within the team.
•  Represent the social work perspective in multi-disciplinary meetings and drive service development initiatives.
• Maintain thorough and accurate records in accordance with professional and organisational standards.

This opportunity requires a degree in Social Work, Social Work England registration, and significant post-qualification experience in adult social care, particularly within mental health or older adults’ services. Your solid understanding of legislation, guidance, and the capacity to supervise and mentor others will be invaluable. Experience in a community mental health setting or additional credentials in mental health or safeguarding is a plus.
