What you will do:
- Manage a caseload that includes completing Care Act Assessments and unscheduled reviews of care.
- Conduct carers assessments and present packages for funding panel approval.
- Engage with community resources to support the independence of service users and their families.
- Perform Mental Capacity Assessments and partake in Best Interest decision-making processes.
- Arrange care home placements and stabilize complex cases for transfer to long-term teams.
- Undertake the initial concern stage of Adult Safeguarding with thorough assessments.
- Collaborate with partner agencies such as the NHS and local housing authorities.
- Work closely with Occupational Therapy staff to enhance social work assessments.
- Innovate solutions to meet the needs of service users and carers, utilising strong communication skills.

Essential:
- Experience with various complex caseloads in OPPD.
- Proficiency in IT databases like Mosaic or Liquid Logic.
- Experienced in safeguarding investigations.
- A minimum of two years' experience post ASYE.

Overview of the Team:
Join a vibrant and fast-paced Assessment and Intake Team that conducts initial assessments for the Older Persons and Physical Disability Service within the heart of the community. This team thrives on short-term engagements of 6 to 8 weeks, carrying out a variety of critical tasks from Care Act Assessments and carer assessments to handling safeguarding concerns and collaborating with occupational therapists.
