What you will do:
Conduct new and follow-up assessments for patients with suspected or diagnosed respiratory diseases such as COPD and Asthma.
Work with patients recently discharged from hospital to provide continuity of care and hospital admission avoidance.
Carry out comprehensive initial assessments, including physical exams, oscillation, NEWS, FeNO test, and jackal assessments.
Prescribe or verify medications, update patient notes, and communicate effectively with GPs.
Schedule and manage follow-up appointments, and refer patients to consultant-led or oxygen-led MDTs as appropriate.
Develop and implement care plans, and discharge patients when possible.
Complete and update all documentation on the EMIS system daily, ensuring thorough record-keeping and referrals to MDTs when necessary.
Experience required:
Must possess substantial experience as a Respiratory Nurse with a preference for those who have worked in community settings.
Ideally a qualified prescriber and adept at performing blood gas analysis, though the latter is not essential.
Training requirements:
Engage in EMIS system training on the morning of your first day, followed by an afternoon shadowing session with an experienced nurse.
