Exciting Career Opportunity: Social Worker within the Mental Health Team in Worcester UK earning £42,186.00 annually. We are seeking a dynamic and experienced social worker to join our Mental Health Team, located in the vibrant city of Worcester. What you will do:
- Conduct comprehensive assessments of individuals' mental health needs and develop personalised support plans in line with the Care Act, Mental Capacity Act, and Mental Health Act.
- Work collaboratively with specialists, including Herefordshire and Worcestershire Health and Care NHS Trust, to deliver effective social interventions and promote recovery.
- Utilise the Three Conversations approach to support individuals by focusing on their strengths and community assets.
- Undertake safeguarding enquiries and employ professional judgement to navigate complex situations.
- Participate in reflective team meetings and supervision sessions to continually enhance your practice.
