What you will do:

• Work as part of an integrated and supportive team, planning, monitoring, and evaluating care with the Multi-Disciplinary Team.
• Conduct needs-led assessments and create care and support plans, handling complex cases with skill and sensitivity.
• Provide guidance and support to adults, carers, and families, promoting self-determination and community integration.
• Participate in safeguarding enquiries and contribute to a quick and personalized safeguarding response.
• Coordinate and lead multi-disciplinary and strategy meetings to ensure effective communication among various stakeholders.
